Strathmore Day Opportunities provides a new approach to the way day services are provided for older people, including those with dementia. We support individuals to lead more active and interesting lives in their community.
The service used to be based at Blairgowrie Community Hospital under the banner of Blairgowrie Day Care with a steady waiting list. In order to provide a service which is based around people rather than around a specific building, and increase the number of people who can benefit, Strathmore Day Opportunities was launched.
You can access Strathmore Day Opportunities by a referral from any of the following:
GP, Nurse, Social Worker, Strathmore Dementia Team, Home Care Organiser, Reablement Service, Occupational Therapist, Perth & Kinross Council Early Intervention and Prevention Team (on AccessTeam@pkc.gov.uk or on 0345 30 111 20).
They then forward your referral to the Locality Community Care Team for assessment. Following the assessment, you will be contacted by staff at Strathmore Day Opportunities.
There are three areas of support provided in Strathmore; Outreach and Carer Support, Community Group Support, or Building Based Support.
We introduce those who are able, to local groups which they will become part of and develop their own social network. We also work with carers to support them to have a life of their own and to prevent ill health. We share information and advise people about relevant opportunities (voluntary, education, recreation etc).
We provide a rolling programme of community based social activities supported by staff. This includes visits to museums, cinema, tea dances, ten pin bowling, lunch clubs, reminiscence groups, garden centres and a variety of other locations depending on service users' choice and aspirations.
We offer structured activities within Harriet Court Sheltered Housing for people who choose this type of support. They include bingo, curling, indoor bowls, cards, dominoes, arts and crafts. Harriet Court tenants are encouraged to participate in the activities along with the service users from Strathmore Day Opportunities.
Service users who opt for community support, depending on the excursion planned have the choice of a healthy packed lunch or lunch out. For service users who are building based, a hot meal is provided.
Strathmore Day Opportunities has a minibus which has a driver and staff member who provides support to service users from their home onto the bus and vice versa. A pool car is also available for transport needs.
